Dose

7.5 to 25 mg weekly (may be dosed higher for treating different conditions); Pediatric dosing 10 mg/m2

Indications

Most commonly used for treating rheumatoid arthritis or psoriatic arthritis, but may also be used for treating lupus or other inflammatory diseases

Side Effects

Nausea, diarrhea, mouth sores, sore throat, reversible thinning of the hair, suppression of blood counts, elevation of liver enzymes rarely leading to liver damage, increased susceptibility to infection, and rare lung damage

Monitoring

Blood count, liver and kidney function studies should be performed at baseline and then every 1 to 2 months while on therapy. Hepatitis testing may also be ordered prior to starting therapy per the discretion of your provider. A baseline chest x-ray may be needed if not done with the past year prior to starting therapy.

Important Points to Know

Contact your provider if any possible side effects occur. Take tablets on a full stomach to reduce nausea. Wear protective clothing or sunscreens to reduce sun sensitive reactions. Avoid or minimize alcoholic beverages, which can further increase the chance of liver damage while taking this medication. Report any increased difficulty breathing or any infectious illnesses to your health care provider. Methotrexate is not for use during pregnancy or while breast feeding. Despite the above side effects, methotrexate has actually been shown to improve survival in rheumatoid arthritis patients in addition to reducing joint damage. Most side effects can be prevented or minimized with proper monitoring.
